About the Role

We are seeking a Nurse Practitioner to help revolutionize how health care is delivered to millions of seniors across America.  As an NP on our Wellness Team, you will work with residents in our partner facilities to ensure that they have access to programs that will improve their health and quality of life. Your work will be a mixture of on-site and telepresence services. 


Responsibilities 

  • Conduct annual wellness visits. You will see patients via an in-person/telemedicine hybrid model to help manage chronic diseases. You will coordinate and collaborate with senior living facility staff to obtain collateral information, including history, vital signs, and the physical exam if remote.

  • Monitor and coach patients with chronic conditions. You’ll use best practices in medicine to help your patients live healthier, high quality lives. This includes remote monitoring of their vital signs and engaging in interventions as necessary. It also means you’ll meet with patients on a regular basis to ensure that they are engaging in healthy actions.

  • Collaborate with other health providers. You’ll work with patients’ primary care providers, the staff at the senior living facilities, and others to make sure that your patients are receiving holistic and consistent care.


Sample Work Plan

During month 1 and 2 you will 

  • Design and refine. You’ll work with our product and operations teams to make sure that tools and processes are in place to help you work effectively and efficiently. 

  • Onboard patients. You’ll visit our assisted and independent living facility partner locations and conduct annual wellness visits with their patients and recommend wellness programs in line with their healthcare goals. 

  • Monitor and coach. You’ll begin to see patients with chronic care conditions for regular coaching sessions. 

During month 3 to 5 you’ll

  • Continue to monitor and coach.

  • Conduct the occasional onboarding visit. As new patients move into our partner facilities, you’ll make sure that they get their annual wellness visits and recommend wellness programs inline with their healthcare goals.

  • Conduct in-person followup appointments. In cases where residents see our telemedicine physicians and a followup call is warranted, you’ll visit the patient in person.


About You

Qualifications. 

You’ll be successful in this role if:

  • You have 5+ years of experience as a nurse practitioner in internal medicine, family medicine, geriatrics, or related field.

  • You are actively licensed to practice medicine in Illinois and/or Texas.

  • You are located in the Chicago or Houston or Austin metro area and can travel within a 1 hour time radius.

  • You can devote least 30 hours a week of available clinical time.

  • You thrive in a collaborative environment.

Suggested requirements:

  • You have 10+ years of experience as a nurse practitioner in internal medicine, family medicine, geriatrics or related field.

  • You have significant geriatric experience or working with a senior population.
